New Student Registration

Please contact Mrs. Davis in the Guidance Office at 639-5594 ext. 52102 to schedule an appointment to enroll your child(ren). 

       

                 The following documentation is required:

                                Completed Registration Form (click here to print form)
                                Birth Certificate
                                Health Records/Immunizations
                                Name/Contact Information of last school attended
                                Custody Papers (if applicable)
                                Free & Reduced Lunch Application (if applicable, click here to print)
                                Proof of Residency, any one of the following are acceptable:  
                                                -Homeowner's Agreement
                                                                -House Deed
                                                                -Mortgage Statement
                                                                -Lease Agreement
                                                                -Property Tax Bill
                                                                -Sale Contract
                                                                -Notarized Landlord Affidavit
                                                                -Utility Bill
